ATHLETIC SEASONS
BASKETBALL - 1st Quarter
Track - 2nd Quarter
Soccer - 3rd Quarter
Volleyball/Football - 4th Quarter
All Athletic Applications/Sports Physical Forms can picked up during your lunch and in the Student Affairs office before or after school, or via the district web site. Applications/Sport Physical Forms must only be complete once and are then valid for the remainder of the school year.
Packets must be turned in to Mr. Harris Only. Packets may be turned in Tuesday & Thursday mornings only from 8:00 to 8:45, and will not be accepted at other times. Students can not practice or participate until packets have been processed and approved.
To Be Eligible for Extramural Sports
Students must have a 2.0 grade point average.
Weekly progress reports must show no “D” or "F" in conduct, and no “D” or "F" in academics.
The following paperwork must be completed before students can begin practice:
Sport Physical, Extramural Application, & 2 copies of the Medical Release form notarized
2. Mandatory insurance coverage
Our insurance program is changing this year (effective Aug. 10, 2009). The online registration link for athletic insurance for the 20011-2012 school year is now available as part of the packet, or you can visit www.schoolinsuranceofflorida.com. |
